1. Gather information and documentation

To begin the process, you will need to provide the following information:

• Personal Information

  Letter describing your current situation and how you would like it resolved.
• If your home is listed for sale, a copy of the listing agreement is needed.

• Financial Information

  Copy of your most recent pay stub from each income source, or other income verification.
• A financial statement form documenting all assets and liabilities (debts) plus monthly income and expenses.

2. Download a documents

3. Return the completed PDF application by mail or fax

4. You can save time by simply completing the On-line Financial Assistance Application instead of following steps 2 & 3 above
