1. Gather information and documentation
To begin the process, you will need to provide the following information:

Personal Information
Letter describing your current situation and how you would like it resolved. - If your home is listed for sale, a copy of the listing agreement is needed.

Financial Information
Copy of your most recent pay stub from each income source, or other income verification. - A financial statement form documenting all assets and liabilities (debts) plus monthly income and expenses.

3. Return the completed PDF application by mail or fax

4. You can save time by simply completing the On-line Financial Assistance Application instead of following steps 2 & 3 above
